This form is to be used in the instance that a customer’s license plate cannot be read or other circumstances, so that they are opted out of reminder notifications about completing their membership enrollment verification form. The customer’s membership enrollment should be completed with standard operating procedures via the Ipad/Tablet if needed i.e. triple stacked license plates or other issues. By submitting a phone number here, it will be excluded from membership enrollment verification reminders/notifications via SMS messaging.
